This notice generally describes how Leal Medical Center may use and disclose protected health information, how you may access it, and our responsibilities under HIPAA and other applicable laws.
Your rights
- Request a copy of your medical and billing records.
- Request corrections when information is inaccurate or incomplete.
- Request reasonable confidential communications by an alternative method or address.
- Ask us to limit certain uses or disclosures, although not every request must be accepted.
- Receive an accounting of certain disclosures.
- Receive a paper copy of this notice.
- Choose someone with legal authority to act for you.
- File a complaint without retaliation.
How we may use or share information
We may use or disclose information for treatment, payment, health care operations, care coordination, legal compliance, public health, prevention of serious threats, workers’ compensation, and other purposes permitted or required by law. Other uses require written authorization, which may generally be revoked in writing.
Special protections
Certain information, including some substance use disorder treatment records, may receive additional legal protection. When applicable, Leal Medical Center will follow the current special requirements.
Our responsibilities
We must protect the privacy and security of your information, notify you following a breach that may compromise it, follow this notice, and provide a copy upon request. We may revise the notice when our practices or the law change.
